Reference Information for Applicants
If you are applying to the Kaplan Leadership Program, please be sure to complete the “My References” form and include it in your application packet. You can download the “My References” form complete with instructions below.

The “Letter of Reference” form is also available below for your references to complete. You can either download and e-mail the form to your references or you can provide a print out of the form to them.
The third document below is the “Nomination” form. Submission of this form is not required, but is recommended. It must be completed by a faculty or staff member at your current college. Note that it cannot be submitted by the person providing your primary academic reference.
